Do NOT use this form if you received an email saying your card is going to expire/is expired. PLEASE contact your home library to renew your expired card.
Your card number will be good for 30 days and can be used for placing holds and accessing online resources.
Contact your library within 30 days with a valid ID and proof of address to complete your registration.
If you are under 18, please bring a parent or guardian with you to pick up your card.
Your default PIN will be your birthday in 6 digit, MMDDYY format. February 5, 1961 would be 020561.
